- Application for transcription of birth certificate (Nuovo-modello-Nascita-1.0-ITA-1)
- Birth certificate of the minor (for South Africa, an unabridged/full birth certificate) legalized with an apostille (for countries that have not signed the 1961 Hague Convention), duly translated with an official translation into Italian (* See “Further information: Legalization and translation”); abridged certificates WILL NOT BE ACCEPTED.
- Photocopy of passports or valid identity documents of parents and minor (if already in possession)
in addition:
- Historical certificate of residence issued by the Italian municipalities where the Italian parent resided (or both parents if both are Italian);
- (Only if the Historical Certificate of Residence indicates residences abroad): Declaration of non-naturalization legalized with an apostille or consular legalization (for countries that have not signed the 1961 Hague Convention), duly translated with an official translation into Italian (* See “Further information: Legalization and translation”), issued by the authorities of all countries where the Italian parents have resided.
With regard to South Africa, the Declaration of Non-Naturalization is issued by the Department of Home Affairs.
- If one of the parents was born in a foreign country or has another citizenship or other citizenships, an original Certificate of Non-Transmission of Citizenship to the Child, legalized and translated into Italian (* See “Further information: Legalization and translation”), issued by the authorities of the country of birth of the parent and/or by those of the country or countries of which he or she has other citizenship;
- Any Certificate of Statelessness issued by the competent foreign authorities of the country of birth of the minor in the original, legalized and translated into Italian (* See “Further information: Legalization and translation”).
